Mandatory Paperwork: Here is a list of paperwork that must be completed and turned in to Kevin prior to Summer Camp:

1) Health form/Physical - Complete Parts A, B & C Please make sure the form is signed by a doctor and a date for the most recent tetanus shot is listed (must be within the last 10 years)

2) Copy of your insurance card (Front and Back) Staple these copies to the completed health form/physical

3) Camper Release form We must have a completed form if your scout will leave camp early. Make sure the name of the person picking up is listed.

4) Range Activities Waiver This must be completed if your scout wants to shoot on any of the ranges (archery, rifle, shotgun, etc)

5) Prescription Medication Form If your scouts needs prescription medication during the week, please complete this form. All medications will be kept in a locked box and administered by an adult.

Swim Check: TBD. We highly encourage any scout that wants to swim or take part in any boating activities at camp complete the swim check PRIOR to camp. Having this done ahead of time drastically reduces the time it takes to check in and allows for more time to set up camp on the first day.
